Gender dysphoria comes in different forms. For me, I was dysphoric about my male presentation, especially my male face, dress and name, and how I was addressed and categorised by others. I wasn’t as dysphoric about my body and genitalia, but it was still dysphoria. For other trans people, it could be the other way around, it depends. Dysphoria manifests differently, but what is constant is the 24/7 sense of wrongness of assigned gender (body at some level).

Trans men on testosterone would clearly have an advantage over cis women who aren’t suppose to take steroids. If it’s good enough for steroids to be banned, then it’s good enough for trans men on testosterone to compete with men under current arrangements, but again, it’s probably time to grow the cake instead: https://blogs.bmj.com/medical-ethics/2019/08/12/transwomen-in-elite-womens-sport-clarifying-the-nuances-of-our-approach/ and https://www.sciencedaily.com/releases/2019/07/190723092111.htm. So yes, more nuance cake, less cheesecake, where the nuance added is more than the cheese taken away. :)
